Step 1 – Fill in the Student’s Application Form

Present the following:

1. photocopy of Form 138 (Report Card)
2. Photocopy of NSO birth certificate
3. 1 pc. 2×2 colored ID picture (blue background)

 

Step 2 – Take the Entrance Examination

 

Step 3 – Undergo Interview (to be scheduled)

1. Initial Interview – Guidance/Designated Personnel
2. Final Interview – Technical Coordinator/Coordinator

 

Step 3 – Enrolment

1}  Bring the following:

1. Original & photocopy of Form 138 (Report Card)
2. Original & photocopy of NSO Birth Certificate
3. Original & photocopy of Baptismal Certificate
4. Good Moral Certificate from your Parish Priest and/or Minister/Pastor
5. Medical Results
6. Chest X-ray
7. Stool
8. Hepa B
9. Drug Test
10. 2 pcs. 2×2 ID picture (blue background)

2 pcs. 1×2 ID picture (blue background)

 

2}  Fill in the Enrolment Form

 

3}  Pay the Registration and Miscellaneous Fees

 

Step 4 – Fitting, Purchasing and/or Ordering of School Uniform

 

1. Fit specified uniform

 

STEM/TVL EPAS & DRESSMAKING = blouse and pants

ABM/TVL Housekeeping = blouse and skirt

ALL Students = MOH-TIW PE T-shirts
